In het HL7 bericht is de eis dat iedere identifier van het datatype II (Instance Identifier) globaal uniek is. Dat wordt gerealiseerd doordat de partij die de II uitgeeft, een unieke OID gebruikt (in II root), en daar de eigen, lokale identifiers aan toevoegt (in II extension). De extension moet dus uniek zijn binnen de gebruikte OID.

Bij een elektronisch voorschrift ligt het voor de hand voorschriftnummers te gebruiken zoals die op een papieren recept ook staan. Deze zijn uniek voor de zorgverlener die het voorschrift opstelt. De applicatie die het elektronisch voorschrift opstelt, dient te borgen dat het voorschriftnummer, dat in extension gebruikt wordt, zowel uniek is binnen de gebruikte root OID als binnen de door de zorgverlener uitgegeven voorschriften.

Bij het tonen van de ondertekende gegevens, wordt alleen het voorschriftnummer, dus de waarde in extension getoond. Dit nummer dient dus uniek te zijn voor de voorschrijver.
