This FHIR profile is based on encounter reports consisting of S-O-A-P encoded journal entries as defined in the HIS reference model (Dutch GP association NHG). Each EncounterReport shall be associated with exactly one Encounter and may be associated with zero to many EpisodeOfCare resources.
Concept
An encounter is between a general practitioner, or someone assisting in his practice and covers one or more issues. The encounter is recorded in S-O-A-P encoded journal entries (Dutch: S-O-E-P). A journal entry may be textual or structured and may additionally be ICPC-1-NL coded. Journal entries are grouped around the issue, so if there are more issues, then the are multiple EncounterReports.
S-O-A-P is:
- Subjective (Dutch: Subjectief)
- Objective (Dutch: Objectief)
- Assessment (Dutch: Evaluatie)
- Plan (Dutch: Plan)
Purpose
Convey information recorded during an encounter.
